If interested in the position please contact Maxim at [removed phone] and ask for Brandon.Position DescriptionThe Independent Living Skills Trainer is responsible for training the participant, participants naturalsupports, andor paid staff, in the types of tools and level of support that allow the participant tobecome as independent as possible in Activities of Daily Living ADLs and Independent ADLs IADLs.Essential Duties and Responsibilities Based on the Functional Assessment, develops and implements a Detailed Goal Plan thatprovides training, supervision, or assistance for the individual, natural supports, andor paidsupports with ADLsIADLs, to include but not be limited to the tools and supports in the areasofo Medication managemento Task completiono Communication skillso Interpersonal skillso Socializationo Sensorymotor skillso Mobilityo Community transportation skillso Reductionelimination of maladaptive behaviorso Issue solving skillso Money managemento Expertise to maintain a household Provides training in an environment and situation which will result in the greatest positiveoutcome for the waiver participant and foster participant independence.Evaluates and reports on the Detailed Goal Plan using an Individual Service Report ISR,submitted to the ILST State Supervisor for review and submission to the participants ServiceCoordinator according to TBI Waiver Provider Manual guidelines Attends the participants scheduled Team Meetings Meets with the ILST State Supervisor for bi-weekly supervision Complies with Maxim of NY Policy and NY TBI Provider Manual qualifications with regards toSerious Reportable and Recordable Incidents Maintains confidentiality of all participants Performs other duties as assignednecessaryMinimum Qualifications Must fit one of the followingo Masters of Social Worko Certified Rehabilitation Counseloro Registered Occupational Therapisto Registered Physical Therapisto Speech-Language Pathologisto Registered Professional Nurseo Masters of Psychologyo Certified Special Education Teachero OR Bachelors Degree and two 2yrs of similar experience, ORAssociates Degree and three 3yrs of similar experience, ORHigh School certificate and four four years of similar experience 1 one year of experience completing functionally-based assessments, developing acomprehensive treatment plan, and teaching individuals with disabilities to be more functionallyindependent Current state drivers license, automobile insurance, and reliable transportation Current health certificate Complete Basic Orientation Training and Service Specific Training per the HCBSTBI ProgramManual Current CPR Certification Clear criminal experience investigation and mandatory registry checks Flexible, yet organized, with the expertise to exercise independent, sound judgment Expertise to effectively elicitprovide info to and from appropriate individuals includingbut not limited to supervisors, co-workers, clients via effective communication abilities Computer proficiency required Location Erie County Compensation CompetitivePrincipals only.
